Physiologic gynecomastia or male breast development can occur as a normal procedure during three phases of your life. Normally it can go away on its own, but if it persists longer than it should, then treatment is needed.

First phase

The first time it occurs shortly after birth in both girls and boys. This occurs because the mother’s hormones stay in the newborn for the first few days after birth which is responsible for stimulating breast tissue growth in the newborn. This can lead to a mild breast discharge, called neonatal milk or also referred to as witch’s milk.

Puberty is the second situation when breast growth in boys can occur physiologically. In fact, up to 60% of boys have clinically detectable gynecomastia by the age of 14 years. Normally the pubertal gynecomastia resolves on its own within three years after the onset.

Third phase;
Gynecomastia of age 

The third age range in which gynecomastia is frequently seen is the older age, commonly more than 60 years. The gynecomastia of age is related to the increased activity of an enzyme called aromatase that leads to an imbalance in your hormones, causing an increase in the levels of estrogen. Elderly people may take certain medications that can cause gynecomastia, and studies show that gynecomastia of age is related to the intake of multiple medications in 80% of cases.

How does gynecomastia look like?

It starts as a little lump under your nipple, and if it does not go away with time, it can grow bigger, causing the nipple to stick out, resulting in puffy and pointy nipples. The breast tissue feels firm and rubbery. Normally it is not too noticeable, but in extreme cases, gynecomastia can look like a women’s breast.

How to check yourself for gynecomastia? 

  • Feel for soft lumps under your nipples on both sides. It can be in one or both breasts, and the size may vary.
  • Check for pain or tenderness, especially when you touch or press.
  • The ability of your nipples to appear forward through the fabric of your shirt is a key sign of gynecomastia.

Can gynecomastia go away with the help of diet and exercise? 

Since the cause of true gynecomastia is the presence of excess glandular tissue, it cannot be resolved with weight loss and exercise; male breast reduction surgery is the only effective way of treating gynecomastia. However, obese or overweight individuals can benefit from losing weight and improve their overall post-surgical appearance.

What is the difference between true gynecomastia and pseudo gynecomastia? 

Both gynecomastia and pseudogynecomastia may look the same in the male chest, but they are different conditions. Male breast enlargement due to accumulation of chest fat occurs in pseudogynecomastia. In contrast, in true gynecomastia, there is an increase in the glandular tissue or breast glands, causing the enhancing the size of male breast tissue.

How to know if you have chest fat or gynecomastia? 

  • In gynecomastia, you can feel a firm lump under your nipples. It gives a firm appearance to your chest.
  • In chest fat or pseudogynecomastia, you don’t feel a lump or firmness in a specific region, rather the fat is soft to touch, and it feels the same throughout your entire chest region. It gives a saggy appearance to your chest.

Can obesity cause true gynecomastia? 

Obesity is a very common cause of gynecomastia in men. In obese men, the levels of an enzyme called aromatase increase, which converts the testosterone into estrogen, causing an increase in the levels of estrogen in your body. These increased levels of estrogen stimulate the growth of male breast tissue, leading to gynecomastia.

How long does gynecomastia surgery take? 

Gynecomastia surgery can be carried out under general anesthesia, and it takes about 1-2 hours, and you can go home the same day. You will need someone to drive you home and stay with you for at least 24 hours so you can recover from the influence of anesthesia.

How long does it take to recover from gynecomastia surgery? 

The recovery time varies from person to person and the amount of tissue removed; generally, you will feel mild swelling, pain, soreness, and bruising for the first few days. You might need to put on a compression garment for a week. If you have an office job, you can return to work after 3-4 days. You will have to avoid heavy lifting and strenuous exercises for 3-4 weeks. Typically the complete recovery time for gynecomastia surgery is 4-6 weeks.

Will gynecomastia surgery leave any scars? 

Gynecomastia surgery results in minimal scarring, with most of them hidden under the armpits. If your male breast reduction surgery is combined with a breast lift as well, you will have some scars around your areola that are well hidden in the pigmentation of your skin.

Can gynecomastia return after the surgery?

Most people are able to get rid of their gynecomastia permanently through surgery, and the gynecomastia does not typically come back. However, in some cases where the cause still remains and stimulates the growth of breast tissue, such as certain medications, herbs, lifestyle or taking steroids, etc., there is a chance of recurrence of gynecomastia.

Can gynecomastia surgery be performed with other cosmetic procedures at the same time? 

Multiple cosmetic surgeries can be performed at the same time as long as you are in good health. Gynecomastia surgery is often combined with a tummy tuck, liposuction of the abdomen and flanks, and body contouring procedures.
